Rheumatoid arthritis (RA) is an autoimmune disease that causes inflammation in the joints.  Autoimmune diseases are illnesses where the body attacks its own immune system.  The autoimmune system is made up of cells that destroy invaders such as infections.  Patients with autoimmune diseases have antibodies in their blood that attack their body tissues that are usually the sight of inflammation; this can occur in organs as well, not just the joints.  RA can affect the eyes, skin, heart, and lungs.  Arthritis means joint inflammation whereas rheumatoid arthritis causes redness, swelling, stiffness, and pain in the joint.  Inflammation in the joints causes swelling, pain, and tenderness to the touch and during movement.  It is possible to have a warm feeling at the sight of the inflammation and restricted movement.  RA can also cause inflammation in tendons, ligaments, and muscles around the joints.  

Rheumatoid arthritis is common in the United States affecting more than one million people and it affects all races equally, though women are three times more likely than men to suffer from this disease.  It can affect someone at any age, but typically occurs after the age of 40 and before 60.  Nobody knows the cause of RA, but scientists suggest it is genetically inherited.  Some environmental factors increase your chances of getting RA such as smoking tobacco.  

When diagnosing rheumatoid arthritis, there isn’t any one test that will give a definitive answer.  Since there isn’t any specific test that will diagnose RA your doctor may order a few blood tests or X-rays.  A couple possible blood tests include the Complete Blood Count (CBC) and the Erythrocyte Sedimentary Rate (ESR or sed rate).  The CBC measures your three types of blood cells: white blood cells which fight infection, red blood cells that carry oxygen, and the platelets that help blood clot.  A low red blood cell count is common among people with RA; this is also a sign of anemia which leads to feeling fatigued.  Your platelet count would be elevated if you have inflammation in your body.  The ESR test measures the rate at which your blood cells fall to the bottom of a test tube.  If you have a high sedimentary rate, it means you have inflammation in your body, the higher the rate the worse the RA.  If your sedimentary rate is high your doctor would have it checked regularly.  If these tests come back with thin normal range and you are still having symptoms your doctor could order X-rays.

There isn’t a cure for Rheumatoid Arthritis (RA), but it can be treated according to each person. The goal of treatment is to reduce inflammation and pain, maximize joint function, and prevent joint destruction.  It is best to start treatment right after being diagnosed.  Aggressive management can prevent work disability, stop damage to joints, and improve mobility.  Aggressive management consists of medications, rest, joint-strengthening exercises, joint protection, and patient education.  Treatment depends on the age of the person, health, which joints are involved, and how progressed the disease is.

Rheumatoid arthritis (RA) is the most common inflammatory form of arthritis, affecting more than 2 million Americans. It is a chronic, systemic, progressive autoimmune process for which there is no cure, currently. However, there are medications available that can put this disease into remission.

Prior to treating rheumatoid arthritis, it is important than any patient suspected of having the disease undergo a thorough and complete examination. This will include an in-depth history, careful physical examination, laboratory testing, and imaging procedures such as x-ray, diagnostic ultrasound, or magnetic resonance imaging (MRI).

While rheumatoid arthritis can affect many internal organ systems such as the eyes, lungs, and heart, its symptoms can be magnified by other conditions.

That is why it is so important that thorough laboratory testing be done. Many of the symptoms of rheumatoid arthritis such as generalized aches and pains in the joints and muscles, fatigue, listlessness, and low grade fever can be due to other conditions.

Anemia, which is frequent in patients with active RA, is a common cause of fatigue.

For instance, it is not uncommon for patients to have an overlap of their rheumatoid arthritis with systemic lupus erythematosus. This condition is referred to as “rupus.”

Also, inflammatory muscle diseases such as polymyositis can often present with joint inflammation along with weakness.

Another organ system dysfunction that is often neglected or overlooked is thyroid disease.

There is an increased incidence of autoimmune thyroid disease in patients with rheumatoid arthritis. When thyroid inflammation occurs, the end result can be hypothyroidism- an underactive thyroid gland. The thyroid gland is responsible for many metabolic functions in the body. When it fails to work properly, symptoms such as cold intolerance, fatigue, lethargy, weight gain, muscle and joint aches and pains can often occur.

Hypothyroidism is significantly more common in female patients with rheumatoid arthritis (RA) than in women in general due to the increased association of these two conditions.

A recent study has demonstrated that this coexistence further aggravates the known increased risk for cardiovascular disease occurring in rheumatoid arthritis patients. (Ann Rheum Dis 2008;67:229-232)

Dutch researchers at the VU University Medical Center, Amsterdam studied 358 RA patients, 236 of whom were women. All were taking part in an ongoing cardiovascular study.

Clinical hypothyroidism was seen in none of the men and 16 of the studied women (6.8%) compared to 2.7% in the general Dutch population. Six of the women had subclinical- meaning unapparent on physical examination- hypothyroidism as did 4 of the men.

The women with clinical hypothyroidism also had significantly more cardiovascular disease than did RA patients with normal thyroid function (37.5% versus 13.0%).

After adjustment for other factors, the odds ratio- the increased risk- was 4.6. In other words, women with RA and hypothyroidism were 4.6 times more likely to have significant cardiovascular disease than women with RA who didn’t have hypothyroidism.

The authors concluded that “clinical hypothyroidism accelerates the already enhanced cardiovascular risk in rheumatoid arthritis and that physicians should consider screening for thyroid disorders in rheumatoid arthritis patients.”

Author’s note: In our clinic, we routinely screen arthritis patients for coexistent thyroid disease. We have found that all too often, unsuspected hypo- or hyperthyroidism is present and once this condition is treated, many of the other symptoms improve as well.

Psoriatic Arthritis

Psoriatic arthritis is a type of inflammatory condition that is said to affect about 10-30 percent of people suffering from the chronic skin condition psoriasis. About 80 percent of people with this condition have psoriatic nail lesions characterized by pitting of the nails. In extreme cases, this condition can lead to loss of the nail itself.

Apart from causing joint inflammation, this disease can lead to tendinitis and sausage-like swelling of the digits. This is referred to as dactylitis. Although this condition is prevalent in people aged between 30 and 50, it can also affect children. Men and women are equally affected by this disease. The exact cause of this condition is unknown but genetic association has been identified as a probable cause.

There are five main types of psoriatic arthritis. The symmetric type simultaneously affects joints on both sides of the body. This type is similar to rheumatoid and accounts for about 50 percent of the cases. Asymmetric does not affect the same joints on both sides of the body. In most cases it involves less than three joints.  

Arthritis mutilans is a severe form and can lead to deformities. It affects about 5 percent of the patients. Spondylitis is another type that is characterized by stiffness in the spine or the neck. It can also affect the hands and the feet. Distal interphalangeal predominant is characterized by inflammation and stiffness in the joints nearest to the ends of the fingers and the toes. Psoriatic arthritis can be treated using joint injections with corticosteroids. It can also be treated using NSAIDs and immunosuppressants.
