Psoriatic Arthritis: More Than Just Joint Pain

What could be worse that this awful joint pain and stiffness, you wonder as you try to will yourself out of bed. While the pain and fatigue that goes along with rheumatoid arthritis can be excruciating and is without a doubt a possibly debilitating condition, there is actually a form of arthritis that is worse. Psoriatic arthritis is a form of the disease that affects not only the joints, but also the skin, and possibly the tendons, eyes, spine, heart and lungs.

In most people who suffer from psoriatic arthritis, the first sign that something is wrong is the inflammation of the skin. Patients can develop psoriasis on their elbows, knees, scalp, and area around the navel and around the anus or genitals. Psoriasis causes red, raised areas of scaly skin. The signs of the skin disease are first seen in the forties or fifties and generally precede joint pain by several months to years. Once the joint pain starts, it generally involves the joints in the feet, ankles and knees. Inflammation can cause the joints to become red, hot and swollen. Often toes and fingers swell so badly they can resemble sausages. Psoriatic arthritis can also affect the spine.

In addition to skin inflammation and joint pain, psoriatic arthritis also attacks the ligaments and tendons of the body. One of the most common tendons to become affected it the Achilles tendon. This Achilles tendonitis causes the patient to have pain when walking and when climbing steps. Along with affecting the tendons in the heels, the arthritis can also inflame the chest wall and the cartilage that links the breast bone and ribs. This inflammation can cause chest pain and shortness of breath.

Along with the joints and tendons, psoriatic arthritis can also affect the major organs of the body such as the heart and lungs. The lungs can become inflamed causing both shortness of breath and chest pain, especially with deep breathing. If the heart becomes inflamed by the disease, the aortic valve can leak. This leak could result in breathing trouble as well as heart failure.

In its rampage on the body, psoriatic arthritis can also damage the eyes. Redness and itching are common results of the condition. The iris or colored area of the eye can also become painfully inflamed, especially when exposed to bright light. Often the only way to relieve this irisitis is to inject cortisone directly into the eye.

Psoriatic arthritis can often be difficult to diagnose especially in the early stages. One common symptom of this form of arthritis, however, is the pitting of finger or toe nails. Sometimes the disorder can cause the nails to turn loose and fall off completely.

Like rheumatoid arthritis, psoriatic arthritis results from the body’s own immune system turning against itself. Unlike the rheumatoid variety, however, psoriatic arthritis can affect not only the joints, but also the skin, eyes and many of the major organs of the body. Left untreated this disease can progress until it become debilitating and sometimes even life threatening.

Researchers at the Universities of Glasgow and Strathclyde will try to understand why auto immune diseases like rheumatoid arthritis and multiple sclerosis are so rare in countries where parasitic worm infections are common and whether this may lead to new effective arthritis treatments.

Yes, that’s right. Parasitic worms living inside the bodies of populations from tropical regions without causing them any trouble at all. And perhaps, strangely, providing some benefit. Which leaves you to wonder if Mother Nature doesn’t know just exactly what she’s doing after all…

The scientific community is well aware of an inverse relationship between worm infections and diseases like rheumatoid arthritis, inflammatory bowel disease, type-1 diabetes and multiple sclerosis. Countries where people are naturally infected with this particular parasite seem to have lower rates of these conditions than those countries where this type of infection doesn’t occur or has been eradicated.

Luckily for the squeamish among us, it’s not the actual parasitic filarial nematode worm that the Scots scientists are wanting to put into our bodies, it’s a large molecule they secrete known as ES-62. This substance is found in the bloodstream of parasite infected people in the tropics, and seems to provide some protective benefit against the inflammation the worms should be causing in their human hosts. The good news is that ES-62 has no known adverse effects, leaving the body perfectly able to fight off other infections.

“ES-62 appears to act like a ‘thermostat’ to effectively turn down disease-causing inflammation while leaving essential defense mechanisms intact to fight infection and cancer,” according to Iain McInnes, a member of the research team and Professor of Experimental Medicine at the University of Glasgow. “This property also makes ES-62 a unique tool for scientists to identifying how such disease-causing inflammation occurs.”

The team is going to try and produce a synthetic (again… whew!) version of the ES-62 molecule from the worms in an effort to come up with an anti inflammatory therapy that works effectively for auto immune diseases like rheumatoid arthritis, inflammatory bowel disease, type-1 diabetes and multiple sclerosis. The three-year study is being funded by the UK’s Arthritis Research Campaign.

Earlier work has also shown ES-62 to have real potential as a therapy for allergies too.

Rheumatoid arthritis is a potentially crippling joint disease that comes as a result of your immune system attacking itself. Inflammation in the joints and internal organs is the result and is an incredibly painful condition.

At present there is no cure for this severely debilitating condition, but there is lots of research currently underway, and various arthritis treatment options are available that help take down the painful inflammation and keep joint damage to a minimum. One such treatment has recently hit the headlines – a drug called Tocilizumab (which will be sold under the brand name RoActemra). This works in conjunction with another arthritis drug Methotrexate and works to halt symptoms.

What is Arthritis? Arthritis is (from Greek Arthro = joint + itis= inflammation) a group of diseases where there is a damage caused to the joints of the body.

There are many form of arthritis : Rheumatoid arthritis , psoriatic arthritis, septic arthritis, Gouty arthritis, osteoarthritis(the most common form of arthritis).

Rheumatoid arthritis and psoriatic arthritis are autoimmune diseases (the body attacking itself)

Septic arthritis is caused by joints infections.

Gouty arthritis is caused by deposition of uric acid crystals in the joints that results in subsequent inflammation.

Osteoarthritis is a degenerative joint disease that can occur after a trauma to the joints or simply aging .

Arthritis is the leading cause of disability in people over the age of 65. By the age of 60 nine in every ten people have it . For most of them arthritis means living with pain and stiffness. However, Arthritis is not an inevitable consequence of aging and can be prevented , provided the underlying causes are eliminated. In the search of causes and solutions many things have been considered including diet, exercise, posture, climate, hormones, infections, genetics ,old age, stress..

The likely factors that lead to the development of Arthritis are:

-Poor lubrication of the joints. In between joints is a substance called synovial fluid. Appropriate nutrition is needed to make sure that the synovial fluid stays fluid and able to lubricate.

- Hormonal imbalance .Hormones control the calcium balance in the body. If the calcium balance is not right, the bones and joints can become porous and subject to wear and tear.

-Allergies and sensitivities. Most of people suffering from rheumatoid arthritis and osteoarthritis have food and chemical allergies or sensitivities that make their symtoms flare up.

- Free radicals . If the immune system is not working properly, as in rheumatoid arthritis, it will produce too many free radicals, which can damage tissue around the joint. In this case a good intake of antioxidant is very valuable.

-Infections. Any infection ,be it viral or bacterial, weaken the immune system which control inflammation.

Bone strain and deformities. Any damage or strain, so often caused by bad posture, increases the risk of developing arthritis .

- State of mind. Research at the Arthritis and Rheumatism Foundation and at the University of Southern California Medical School has shown a link between arthritis and emotional stress.

- Poor diet . Most people with Arthritis have a history of very poor diet, which pave the way to many of the factors stated above.

Diet advice: avoid sugar and refined carbohydrates, stimulants, fat ,too much protein . Drink plenty of water and herb teas. Make sure you have all the vital vitamins, minerals and essential fatty acids, antioxidants nutrients.

Research trials are conducted for all pharmaceutical drugs for arthritis that eventually reach the market.

A clinical research trial may be the only way you can get early access to a breakthrough arthritis treatment; however, there are pluses and minuses so it’s important to understand the questions that need to be asked and answered.

It should be emphasized that a clinical trial is just another component of standard arthritis treatment. It should be considered as an option for patients receiving conventional care.

Clinical trials are research studies that evaluate a new drug or treatment in three separate phases, prior to FDA approval. In a Phase I trial, the safety and the dosing level of a new drug are tested in a small group of patients. The trial examines the pharmacologic action of the drug in humans and its side effects at different dosage levels.

Often, the earliest subjects in a phase I trial are normal volunteers.

In a Phase II trial, the drug is further tested for safety and for efficacy in a larger group of people who have the disease under study.

Once efficacy has been established, the drug is tested in a Phase III trial among several hundred to thousands of patients to further explore its safety and efficacy and whether or not it is better than conventional treatment for arthritis.

Phase III trials attract the most attention among arthritis patients; however, not everyone can be in a trial, nor are they guaranteed to get the drug being tested. To be in the trial, patients must first meet entrance criteria, which are very specific and which exclude many patients. If they are accepted, patients may be randomly assigned to different treatment groups, depending on the phase of the trial.

In a Phase III trial, the “test” group receives the new drug, while the “control” group receives either the standard treatment for that specific type of arthritis or a placebo (an inactive pill, liquid or powder). In some trials, called a “single-blind” study, researchers are aware of who is taking what drug, while in others, called “double-blind,” neither the participants nor the research team knows what the participants are receiving.

Sometimes trials are stopped early, for a variety of reasons. One possibility is that one treatment is found to be vastly superior to the other, while another is exactly the opposite — a treatment is found to be dangerous. When trials end early because of positive results, the patients receiving the beneficial treatment will continue to get it, and also, the rest of the patients in the trial are offered the drug as well.

Obviously, if the results are negative, the drug is immediately stopped in all patients.

Make sure you see an expert rheumatologist. It may be worth traveling some distance to find the best expert for your arthritis condition.

Educate yourself. Read everything you can about the center and the trial.

Questions to ask:

• How long has the center and the rheumatologist been conducting research?

• Have there been other studies that evaluated this drug, and if so, what were the results?

• How do the risks, side effects and benefits compare with my current treatment?

• What are my odds for receiving a placebo if I participate in the study?

• Who is the research team that will take care of me during the study?

• Will involvement require an extra time or travel commitment on my part… and will I be reimbursed for expenses?

• How might the study affect my daily life?

• How will I be informed about new risks that may be identified during the trial?

• If the treatment is working for me, can I continue getting it even after the study ends?

• What are my costs associated with the trial? Will my insurance cover the costs? If not, what will it cost me?

• Are there other patients already taking part in the study whom I could speak to?

• Will I be able to find out about the results of the study at the end?

Research clinical trials are an option for getting first crack at new drugs, saving money, and also for helping to make a contribution to medical research and to the future health of others with arthritis.
